Support Coordination may also include help with life stage transitions, supported accommodation discussions, in-home care arrangements, respite, short-term accommodation, medium-term accommodation and coordination with multiple providers.
Support Connection is a lower level of support that helps participants build their ability to connect with informal, community and funded supports. The goal is to help participants get the most out of their plan and work towards their goals with more confidence.
Support Coordination is a medium level of support that helps participants build the skills needed to understand, implement and use their NDIS plan. A Support Coordinator works with the participant to ensure the right mix of supports are in place to improve independence, maintain relationships, manage service delivery and participate in the community.
EC Care Support Services prices are in line with the NDIS Price Guide and services are delivered in accordance with the NDIS Code of Conduct and Practice Standards.
